Attached to this article is our Request for Owner Information Packet.  There are a couple basic documents such as the Property Management Agreement, and W-9, as well as several commonly used CHA documents. We prefer to collect this basic information upfront so that we can move quickly should you have a CHA tenant.  


If you are a new owner who purchased a property from Elite Invest, LLC, you do not need to submit a copy of the recorded deed, HUD/Master Statement, or other closing docs, we are able to obtain those from our attorney. 


The Housing Assistance Payment (HAP) Contract Assignment is the document that we submit to CHA and allows us to act on your behalf. Please sign next to Alex's signature. 


Please fill out and execute the Property Management Agreement (PMA), be sure to provide an emergency contact and full address information on the space provided. The term of the PMA will be 1 year, with the start day the date of your closing. If you are transferring management it will be one year from the date of transfer. Be sure to notate all items listed on "Exhibit B", it explains exactly what owners are responsible for and what charges you should expect to see on your owner statement. 


The Authorization For The Release of Information--Owner document authorizes CHA to obtain personal information to verify that you are eligible to participate in the Chicago Housing Authority's (CHA) , Housing Choice Voucher (HCV) program. 


W-9, the information provided on this form is what we will report to the IRS at the end of the year.  If you move this property into an LLC at a later date this form must be updated, otherwise we will report to the IRS whatever information we have on file.
